Nurse Practitioner Sarah Tamayo-Perez coordinates Cucamonga Valley Medical Group Diabetes Education Program

Cucamonga Valley Medical Group is more than a place where patients can count on receiving high-quality care. We’re also a community partner. We’ve embraced each of these roles in launching our six-week Diabetes Education Program coordinated by CVMG Nurse Practitioner Sarah Tamayo-Perez.

The first of six 90-minute classes offered at no cost to CVMG Spanish-speaking patients was held on Thursday, Oct. 11. The ongoing classes will begin at 6 p.m. each week at CVMG’s North Fontana location at 16465 Sierra Lakes Parkway, Suite 300, Fontana.

Certified Diabetes Educator Noe Lopez, RN, and Maria Rojas, Hispanic community educator are instructors during the classes. “The classes are a valuable asset available to patients,” said Rahmi Mowjood, D.O., of Cucamonga Valley Medical Group.

“These classes will include information about disease management, life style changes, risks and overall efforts to help control and manage this condition.”

Twenty-five qualifying patients enrolled in the first class. Educational materials provided by the American Diabetes Association will be utilized in the classes throughout the program. “We have full confidence in Noe Lopez and Maria Rojas’ abilities and creativity in presenting the material,” said program coordinator Tamayo-Perez. “We are excited about this opportunity to help our patients better understand and manage this disease.”
