  • Lauren Crostic, PA

    Lauren

    Born and raised in the Inland Empire, certified Physician Assistant, Lauren Crostic is excited to give back to the community where she grew up. Mrs. Crostic joined CVMG in 2016 after finishing her Master’s degree in Physician Assistant Studies at Western University of Health Sciences in Pomona, California. Prior to her graduate education, Mrs. Crostic attended UC Riverside where she obtained a Bachelor’s degree in Biology. While working towards her undergraduate degree, she worked as the Lead Scribe of the emergency department at San Antonio Regional Hospital. She also volunteered on a medical mission abroad to Honduras and dedicated time to the Student Run Health Clinic at UC Riverside providing free healthcare to the population.

    Mrs. Crostic considers patient education to be an essential part of her practice, as well as being an advocate for each patient’s medical care. She recognizes that no patient is the same, and therefore enjoys partnering with them to ensure they are receiving the best individualized care they deserve. In her spare time, Mrs. Crostic enjoys spending time with her family and husband, traveling the world, baking and yoga.

  • Yumi Wong, PA

    Yumi

    Ms. Yumi Wong is a licensed and certified Physician Assistant who joined CVMG in November 2016. She completed her Bachelor of Science in Physiological Sciences at UCLA and went on to continue her education at The George Washington University in Washington, D.C. There Ms. Wong earned a dual Masters degree in Health Sciences in Physician Assistant Studies and in Public Health with a concentration in Community Oriented Primary Care.

    Growing up in Rancho Cucamonga, she returned to the Inland Empire after her studies to begin her medical career in providing quality healthcare to the local community. With over a decade of experience in diverse medical settings, Ms. Wong’s dedication lies in family practice where she continues to provide comprehensive and compassionate care to patients with chronic and acute conditions.

    Outside of the medical setting, she is an avid runner, completing numerous half and full marathons each year in search of the next best sunrise and adventure. She also enjoys hiking, baking, arts and crafts, gardening and spending time with her family and two dogs.

  • Tameem Sabry, PA

    Tameem

    A Southern California native, Physician Assistant Tameem H. Sabry is excited to follow his family’s legacy of treating patients through Family Medicine. Mr. Sabry comes from a family of Emergency Medicine physicians who practice in Southern California. Mr. Sabry, a former business executive now redirected into healthcare, received his B.A. in Communications from California State Fullerton and later went on to earn a Master’s in Business Administration. After a successful career in executive business management, Mr. Sabry felt his calling was in the service of others and rediscovered his passion for medicine through teaching Emergency First Response, underwater rescue and CPR as a PADI Open Water Scuba Instructor. He went on to become an EMT prior to attending Touro University Nevada. During his time at Touro University Nevada as a PA student, Mr. Sabry flourished as an advocate to the local underserved population, where he spearheaded the creation, implementation, and ongoing development of Nevada’s first ever Mobile Clinic designed to deliver healthcare to the homeless and underserved populations of Southern Nevada. As a student, he received multiple regional and national recognition, including winning the inaugural PA Foundation IMPACT Grant, The Tony Hsiesh (owner of Zappos) Downtown Project Scholarship for Service to the Underserved, and the Touro University Nevada Service to the Institution Award. Mr. Sabry is passionate about listening to, and advocating for his patients. Mr. Sabry aims to provide exemplary patient satisfaction and patient outcomes while building a positive rapport with his patients.

  • Sarah Perez, NP

    Sarah

    Raised in the Inland Empire, Ms. Sarah Tamayo-Perez, a board-certified Family Nurse Practitioner, enthusiastically seeks opportunities to positively impact her local community. As such, she is proud to be a part of the patient-centered team at Cucamonga Valley Medical Group. After completing a Bachelor’s of Science in Nursing at Azusa Pacific University, she dedicated five years to patient care as an emergency department nurse at San Antonio Regional Hospital. Seeking to nurture both her passion for medicine and her dedication to patient-care, Ms. Perez pursued a Master’s of Science in Nursing with a Family Nurse Practitioner specialty at Frontier Nursing University. She completed her nurse practitioner clinical training at multiple family practices and acute care settings in the Inland Empire. She is a member of the California Coalition of Nurse Practitioners, the Sigma Theta Tau Nursing Honor Society, and the National Catholic Bioethics Center.

    Ms. Perez views the patient-provider relationship as one of mutual respect and collaboration.
    Her goal is to see the individual patient as a holistic person within a community and family unit and to use this perspective to help persons achieve their health goals. She is committed to integrating her clinical expertise, the use of up-to-date medical research, and a caring, joyful spirit at each patient encounter.

  • Eric Bekemeier, DO

    Eric

    Dr. Eric Bekemeier is excited to join CVMG and bring his passion for primary care to the local area. Originally from Detroit, Michigan, Dr. Bekemeier attended Michigan State University where he obtained a Bachelor’s Degree in Human Biology, and a love for Spartan football. From there he moved to California to start his medical school training at Touro University College of Osteopathic Medicine in the San Francisco area. Dr. Bekemeier met his wife, our fellow physician, Dr. Kimberly Bekemeier in San Francisco and they subsequently moved to the Southern California area to start residency training. He trained at Pomona Valley Hospital Medical Center-UCLA, obtaining his Family Medicine Residency degree.

    Dr. Bekemeier started his practice in the High Desert and is happy to now be joining CVMG closer to home in the Rancho Cucamonga area, to help you and your family. He cares for patients of all ages, from birth onwards, and can help you manage all your basic health needs from the common cold to your chronic diabetes.   • Kyle Smart, DO

    Dr. Kyle Smart is committed to providing excellent medical care combined with unparalleled “customer” service. This is represented by a caring attitude at all times by every member of the CVMG staff. Dr. Smart’s extensive experience includes primary care, physical exams, and disease management for infants to seniors and all ages in between. When necessary, Dr. Smart refers patients to only those qualified specialists that he knows and trusts.

    Dr. Smart completed his residency training at Arrowhead Regional Medical Center in Colton, California. He served as House Staff President during the completion of his training, and he currently serves on committees at both San Antonio Regional Hospital and PrimeCare of Inland Valley. Representing the third generation of his family to live and work in the Cucamonga Valley, Dr. Smart has strong ties to the local community and has been a staff physician at San Antonio Regional Hospital since 2005.

  • Natalie Herrera, PA

    NATALIE

    Ms. Herrera considers service and humility to be the cornerstones of her health education and career, allowing her to continue to define her purpose of helping and healing. She strives to treat each patient individually by creating a comfortable health care environment and providing top quality medical care. Ms. Herrera joined CVMG in 2015 after graduating from Keck School of Medicine of USC, where she obtained her Master’s degree in Physician Assistant Studies. Prior to her graduate education, Ms. Herrera studied Biological Sciences locally at California State University San Bernardino. While working towards her undergraduate degree, Ms. Herrera also worked as an emergency department scribe at San Antonio Regional Hospital and Barstow Community Hospital. During this time, she volunteered to educate abused and homeless women at Veronica’s Home in San Bernardino, offering free health and wellness classes. After moving to Los Angeles from the Inland Empire to further her education, Ms. Herrera became heavily involved in volunteering with the medically underserved communities of Los Angeles. As Vice President of her class and an executive board member of the USC student run clinic, Ms. Herrera assisted in managing free clinics in downtown Los Angeles, LAC + USC Medical Center and helped establish the first USC free clinic at skid row. For her service and leadership, Ms. Herrera was awarded Pi Alpha graduate honors and continues to pursue service to the community as part of her professional career. Although Ms. Herrera’s hobbies include hiking, bike riding and painting, she considers music to be her lifelong passion.

  • Shina Patel, PA

    Ms. Shina Patel is a certified Physician Assistant, and has been a valuable member of Cucamonga Valley Medical Group since 2007. She completed her undergraduate degree at California State University, Long Beach in Healthcare Administration with emphasis in Information Systems and Marketing. She then received a Master’s as a Physician Assistant from Western University of Health Sciences in 2005. Her focus is in Family Practice, Urgent Care and Women’s health. She enjoys working as a Physician Assistant and feels that patients need to be treated as a whole. The body and mind need to be in alignment to help resolve medical issues. With today’s high stress life style, health eating, mind and body are very important.

    In her spare time she enjoys spending time with her family. She enjoys travelling and experiencing all the colorful cultures out there. Travelling has helped her connect with some of her patients from around the world. She enjoys dancing, cooking and watching movies.

  • Jesse Dominguez, DO

    Having been raised in a small town, Dr. Jesse Dominguez understand the value of personal relationships, a characteristic appreciated by his patients, as well as the CVMG office staff. A family practice-trained physician, Dr. Dominguez knows that medicine is an ever-changing field. His continuing education comes not only from seminars and journals, but also by learning from his patients as he listens to their concerns and treats them with expert skill.

    Prior to beginning his medical education, Dr. Dominguez spent eight years working as a correctional officer for the State of California. Realizing that his childhood dream of becoming a physician was too important to give up, he moved to southern California to attend California State University San Bernardino. After graduating in 1997 with a Bachelor’s degree in Biology, Dr. Dominguez attended medical school at Western University of Health Sciences in Pomona, California, and completed his residency at Natividad Medical Center in Salinas, California. Dr. Dominguez has been a great addition to Cucamonga Valley Medical and is a staff physician at the renowned San Antonio Regional Hospital.

  • Rahmi Mowjood, DO

    RAHMI

    Your health and well being are of the upmost importance to Dr. Rahmi Mowjood. While he sees patients of all ages, much of his time is spent in the clinic educating our older patients on long-term care and other issues affecting their health.

    After Dr. Mowjood graduated from the Webb Schools in Claremont, California, he received his Bachelor’s degree from Claremont McKenna College in 1994. The following year, Dr. Mowjood was awarded a Rotary Ambassadorial scholarship by Rotary International, sponsored by the Upland Rotary Club, which allowed him to study in London, England. He went on to graduate from Western University’s College of Osteopathic Medicine in 2000, and he completed his residency at Arrowhead Regional Medical Center in 2003. Dr. Mowjood teaches at Western University, previously as an assistant professor of family medicine, and currently as an adjunct professor.

    Dr.Mowjood is also a member of Medical Safety and Management, Inc, and covers many southern California sporting and entertainment events, including events at Angels Stadium.

  • Kimberly Bekemeier, DO

    Kimberly

    As she joins our staff at Cucamonga Valley Medical Group, Dr. Kimberly Bekemeier, a native southern Californian, is proud to provide care to people in her own community. Dr. Bekemeier attended UC Riverside, where she obtained a degree in Biology, and then attended Touro University of Osteopathic Medicine in the San Francisco bay area. She returned to Southern California for her residency training at Pomona Valley Medical Center, where she headed the Women’s Health track, focusing on compassionate care for a variety of women’s health issues. She continues to bring her passion for treatment and prevention of women’s health to our clinics, along with her knowledge of a wide variety of chronic disease states such as diabetes and high blood pressure.

    An active mom herself, Dr. Bekemeier understands the challenges of maintaining optimal health in our day to day life. She is eager to assist you and all members of your family, to prevent disease before it happens and to ensure that each patient has the quality of life they desire.

  • Michelle Viray, PA

    Michelle

    Returning to Southern California, Physician Assistant, Michelle Viray, is excited and dedicated to serve and provide care to the Inland Empire. Ms. Viray attended UC San Diego, where received a degree in Biochemistry. She then attended the Joint MSPAS/MPH program at Touro University – California and earned masters degrees in both Physician Assistant Studies and Public Health. With her background in family medicine and public health, she continues to promote health and the well-being of our community by focusing on disease prevention and health promotion.

    She is passionate about patient empowerment through education and awareness. By building strong patient-provider rapport, she believes we can prevent disease, better understand our health, and achieve a quality of life we all strive for.
