Diabetes Self-Management Education*
& Support Program

The Centers for Disease Control and Prevention report that approximately 1 in 3 American adults (88 million) have prediabetes while over 1 in 10 (34.2 million) have diabetes.

 

Diabetes Self-Management Education* & Support Program

Cucamonga Valley Medical Group has been recognized by the American Diabetes Association for Quality Diabetes Self-Management Education* and Support. CVMG is proud to offer this program to adults 18 and older who are living with diabetes, have been newly diagnosed, have uncontrolled symptoms, or want to make major changes to reduce or eliminate the need for medication.

Freedom From Smoking

According to the American Lung Association, when burned, cigarettes create more than 7,000 chemicals, 69 of which are known to cause cancer.

CVMG’s Freedom from Smoking Clinic is an American Lung Association accredited program intended to help adults 18 years and older learn the proven skills and techniques to quit smoking.

Healthy Living Program

According to the U.S. Department of Health & Human Services typical American diets exceed the recommended intake levels or limits in four categories including calories from solid fats and added sugars; refined grains; sodium; and saturated fat.

 

Healthy lifestyle choices are at the core of this monthly program. Attendees will learn more about basic human physiology and receive useful tips and evidence-based advice to make more informed decisions to improve their health.

Patients of all ages are welcome to join these interactive sessions as well as participate in the live Q&A to ask questions specific to their circumstances.

Sabrina Tupuivao, Community Health Educator at Cucamonga Valley Medical Group, is a healthy lifestyle advocate and educator who applies her experience and knowledge to help others make healthy choices for living. Sabrina received a Bachelor of Science in Health Science from California State University San Bernardino where she strengthened her belief in the importance of a healthy lifestyle and educating others in evidence-based health information. She also teaches a Diabetes Education Class, supporting patients in finding ways to manage their diabetes and live a full life. Through mentoring others in health behavior change, Sabrina has witnessed the happiness that comes from healthy lifestyle choices and is committed to helping others live their best life.
