Drive-Thru Clinic will be held in the parking lot at: Cucamonga Valley Medical Group – North Fontana 16465 Sierra Lakes Parkway • Fontana, CA 92336
- Open to both existing patients and the community at-large for children ages 4 – 21
- $20 Fee (cash or card) or private insurance plan required regardless of number of vaccines received (Patients who have coverage through MediCal or other Government Benefits will need to schedule an office visit.)
- The child’s yellow Immunization Record must be provided or CVMG will not be able to vaccinate them Patients (children ages 4 – 21) and parents must wear masks and remain in their vehicle for the entire process.
