CVMG providers support healthy habits for all patients, young and old.  We know you want to do your best to keep you and your family healthy.  CVMG Bingo 2019 is the place to start!
This game is a fun way for you to track your health and wellness habits as well as mark off completed recommended screenings and tests.  Here’s to a healthier YOU!

 

How to Play the Game:

Get your CVMG Bingo card on your next visit to your CVMG provider.  CVMG staff will punch each square you complete throughout 2019.  You must complete all 16 squares to win at CVMG Bingo.

CVMG 2019 Bingo started January 1, 2019 and ends December 31, 2019.  When you have completed all 16 squares, bring your card into a CVMG office for verification.  Everyone with a completed, verified Bingo card wins a Prize!

You and your children will have until January 31, 2020 to return your completed cards to a CVMG office and claim your prizes.

If you don’t have an upcoming visit scheduled, you can still start to play today.

 

Choose either the adult’s Stay Healthy 2019 card (for adults age 18 and older) or the children’s Healthy Kids card (for children 0 – 17).  Print a temporary card now, and mark off the squares you and your children have already completed in 2019. 

Take the temporary card with you on your next CVMG office visit, and trade it in for your CVMG Bingo card, which will be verified and updated to match the squares you have already marked as completed.

Adult Stay Healthy (ages 18+) Square Completion Guidelines:

 

 

Controlled Glucose All adults 18 and older need to have their glucose screening with fasting labs within the past 12 months.  If you are a Diabetic, you must also have a A1c of less than 7 to complete this square.

Controlled Cholesterol To complete this square you must have a fasting lipid panel with LDL at goal of less than 130 within the past 12 months.   If you are a Diabetic or a Heart patient, your goal for the same time period is less than 100.

TDAP Vaccine Your TDAP vaccine must be documented as administrated in a CVMG office or you must have proof from an outside facility within the past 5 years.

Colonoscopy All adults 50 + are required to have a colonoscopy documented by a GI within the past 3 – 10 years, or have completed an FOBT (Fecal Occult Blood Test) within the past 12 months.  If you are <50, this is a FREE square for you!

No Smoking To have this square punched you must have a negative social history.

Pap Smear / PSA >40 Females ages 21 – 29 must have a pap smear within the past 3 years; Females ages 30 – 65 must have a pap smear within the past 5 years.  Males who are 40 – 65 yrs must have PSA lab/blood work in the past year.  If you are a male < 40 years, this is a FREE square for you!

Annual Physical You must have an annual exam performed within the past 12 months to complete this square.

Flu Vaccine 2018-2019 Immunization given in CVMG office or written proof brought in from outside source (i.e. Pharmacy slip or from employer)

Mammogram / Self breast or if <40 Testicular Exam If you are a 40+ female you must have record of a Mammogram within the last 12 months; OR if you are a <40 female you must have received provider education on importance of monthly self breast exam and acknowledge that you know how to perform the examination; OR if you are a male 18 – 40 you must have provider education on importance of monthly self testicular exam and acknowledge that you know how to perform the examination.

Exercise 3-4 times a week You are encouraged to exercise 4 times weekly, a mix of 20 – 30 minutes of cardio and 2 days of strength training (weight lifting), and have a plan in place.  Your plan must be identified and documented during an office visit.  If you are not exercising, an Rx for exercise will be discussed and documented.

Eat 3-5 servings of Fruits & Veggies a day Before you can complete this square, your physician must discuss with you the importance of consuming 3-5 servings of fruits and veggies daily and document your food plan on your patient chart.

Controlled Blood Pressure Your blood pressure must be less than 130/90 and be documented during an office visit.

Web Enabled at CVMG If you are not already web enabled to use the CVMG Patient Portal, you can do it right now!  Call CVMG at 909-429-2864 and ask to be web-enabled.  The office staff will provide your login credentials and get you started.

Wear Sunblock daily Your card will be punched to complete this square once you acknowledge with your physician that you know how & when to use sunscreen and how to check your skin.  During a visit, your provider will educate you on the importance of wearing SPF 15-30 daily, avoiding peak exposure times, and using protective clothing.  You will also learn how to do your own monthly skin exam and check moles using the ABCDE’s (Asymmetry, irregular Borders, changing Color, increasing Diameter, Enlarging)

Annual Diabetic Eye exam / Optometry exam If you are a diabetic you will need a consult  from a trained optometry or ophthalmology eye care professional within the last 12 months.  All other adults should be educated on the importance of eye health.  Your card will be punched when you either have documentation of a completed optometry appointment outside of CVMG or have your visual acuity checked in office and a referral as needed to an outside eye care professional.

Annual Dental Exam Your card will be punched once you acknowledge with your physician that you understand the importance of routine dental cleaning every 6 months.

 

 Children’s Healthy Kids (children 0 – 17 yrs) Square Completion Guidelines:

Annual Eye Exam if 3+ Children ages 3 – 18 yrs should have a visual exam performed in a CVMG location at an age appropriate well child physical within the past 12 months.

Prevnar Vaccine, Hepatitis B Vaccine, Hepatitis A Vaccine, DTAP Vaccine, Polio/IPV Vaccine, Hib Vaccine, Meningitis Vaccines for 12+ To complete these vaccine squares, a child must have an immunization given in office, proof of a vaccination given with previous medical office records, a school immunization record, or a yellow vaccine card to verify the age appropriate vaccines.  If a specific vaccine is not appropriate for the child’s current age, this is a FREE square!

TDAP for 10+ Children 10+ yrs are required to have a on-time TDAP booster dose vaccination given in office, proof of the vaccination given with previous medical office records, school immunization record, or yellow vaccine card to verify.  If the child is less than 10 yrs, this is a FREE square!

Annual Dental Exam for 1+ Once parent/chid discuss dental health with provider and acknowledge the importance of routine dental cleaning every 6 months starting by age 3, this square can be punched.

Exercise / Stay Active Child and parent will be educated on the importance of 20 minutes of daily activity every day of the week, limited screen time, and limited sugary foods and drinks.  Once educated by provider, square is completed.

Eat Veggies & Fruits Before this square can be completed, child and parent must discuss with provider the importance of consuming 3-5 servings of fruits and veggies daily and document a food plan on the patient chart.
